The Thames Valley and Surrey Care Records is a way of bringing together and sharing patient information from across individual local care records with health and care staff over a wider area, so they can deliver better care for you.
As we work to create this and begin to connect with data in different local care records, we will share with you the current status of the progress that we make below.

TVS Care Records - status updates

Acute data from Buckinghamshire is feeding into the TVS Care Record. GP data from EMIS practices in Berkshire West and Frimley is also flowing into TVS Care Record, therefore patient information from four partnership areas is populating into TVS.

Buckinghamshire GP and social care (adults and children) is in the TVS Care Record. Project started for the technical requirements for Buckinghamshire My Care Record professionals to view the information in the TVS Care Record. See more information about the data coming from the organisations.
Now that we are close to sharing data from more than one area in the TVS Care Record, we need to ensure that health and care professionals are able to access the data. For example, staff working in Buckinghamshire need to be able to access data shared from Surrey. A project team is working on this goal. Technical developments are also being undertaken to the TVS Care Records platform to enable the sharing of e-forms and care plans. This will allow professionals to know the wishes and choices of the patient

Together with health and care teams across the region the TVS programme has now developed an online dashboard which will display information to health and care professionals about those people who make use of health services frequently - A&E, non-planned admissions, primary care, 111, ambulance and mental health services. These people are sometimes referred to as High Intensity Users (HIU) as they often have complex health needs which require support from multiple health services.

The Surrey region is connected to TVS Care Records. This allows the Surrey data to be shared with others in the TVS area and includes data from GPs, the acute hospital, social care (adults and children’s) and community health.

The TVS Analytics platform is fully functional, able to take data for direct care and population level health information. Currently used by Surrey it offers a range of information in different formats to support care.
