The Thames Valley and Surrey Care Records Partnership has signed a contract with the System C and Graphnet Care Alliance to provide a new region‐wide health and care record and population health system. Our aim is to drive improvements in care by building on and joining‐up existing local care records across the region.
A range of software and services will be put in place to enable the safe and secure sharing of an individual’s health and care information as they move between different parts of the health and care system within Thames Valley and Surrey. It will also enable an individual’s record to be shared for specific care purposes if they need treatment further afield.

This will create an environment for sharing information to help our health and care services continually improve the treatments we use, from ensuring that care is tailored to the needs of each individual, to helping people to look after themselves better and make informed choices about their own health and care. Patients receive care in places and organisations that cross the Thames Valley and Surrey area, and we need the digital infrastructure to support this.
The system will use open standards to enable a range of digital services and applications and services to work with it. It will not replace the existing local care records (such as Share Your Care in Berkshire and Frimley and My Care Record in Buckinghamshire), but instead will link the existing local care records together and appropriate information governance and data sharing arrangements will be established to support implementation.
